01Output record
- Question as asked, and the question as interpreted by the system
- Sources intended versus sources reached
- Oldest source used and whether that age is acceptable here
- Claims with no supporting citation, listed explicitly
02Verification
- Two claims spot-checked against their cited excerpts
- Any conflict between sources noted and preserved in the output
- Any claim the system should have refused, flagged
03Decision
- Reviewer name and review date
- Publish, revise, or reject, with a one-line reason
- Follow-up needed before this output can be reused
